North Penn High School Online Registration Procedures for Spring 2024
Parents/Guardians,
There is an update to the process from last year. The athlete’s completed PIAA physical MUST be uploaded to the online registration in order to finish the registration!
**Log onto FormReleaf.com**
If you registered last year:
Log In with your information from last year. Go to "Find Organization" and type in North Penn. Click on "North Penn” then scroll down to “Programs” and click on “2024 Spring Athletics”. Please choose a sport and scroll down to “Participant Information” Please choose a sport and scroll down to “Participant Information” and select “Auto Fill” for the student athlete for whom you are registering. The data will then be filled in but you must sign each item at the bottom, which will recertify your child for the fall. Once you have done that, “Submit” will appear at the bottom and after you click that, your student athlete will be registered for their fall sport. To confirm the registration, you will receive a confirmation email.
If you did not register last year:
Please "Sign Up" (in the upper right corner) and create a profile then Log In. Go to "Find Organization" and type in North Penn. Click on "North Penn” then scroll down to “Programs” and click on “2024 Spring Athletics''. Then, choose a sport and scroll down to “Participant Information” and fill out each field. Required fields have a red star. When finished, click "Submit" located in the lower right hand corner. You will receive a confirmation email that it was completed. If you do not receive an email, please go back through and find what you are missing and resubmit.
******PHYSICALS MUST BE DATED JUNE 1, 2023, OR AFTER, to be valid for the 2023-2024 school year. They are then valid until May 31st, 2023 or the conclusion of the spring sports season.******
Impact Testing
Every athlete that plays an impact sport must take the ImPACT test EVERY 2 YEARS, before their season begins. The test is taken at home, at your convenience and should not be done after a vigorous workout or on a phone. It should be done in a quiet area and it will take about 30-45 minutes.
To access the ImPACT test, complete the online registration mentioned above. Once that registration is completed, you will receive an email from ImPACT within 72 hours. The email will be sent to the email address used to complete the online registration ONLY if you need one. If you took an ImPACT test last year, you will not need to take another one and will NOT get a link emailed.

PHYSICAL, ONLINE REGISTRATION and IMPACT TESTING ARE ALL DUE ON FEBRUARY 23, 2024. These dates are to ensure paperwork is filled out properly and everything is turned in before preseason starts. If ANYTHING is missing, the athlete WILL NOT be allowed to participate on the first day!
